• Discharge Authority Form (PDF, 106KB) (you must have your Mortgage Manager send this form to us, if your loan is not with PLANLend, ChoiceLend, FASTLend, AFG Edge, Connective Essentials, Mortgage Choice Home Loans SmartSelect, Astute Simplicity, Go Edge, MyLoan Elect, LJ Hooker Home Loans Connect, Yellow Brick Road Home Loans Select and Resi Select). IMPORTANT: Your request will not be accepted if this form is not received from your Mortgage Manager.
